COPD Education Clinic

Objective

Provide COPD Education to patients in the Cold Lake area with the aim to improve:​
• Disease understanding
• Self management skills
• Quality of life and to decrease exacerbations

Visit Descriptions

Visit One

​• Assess stage of change, receptiveness, level of concern
• Identify patients main concern(s) and ensure they are addressed at this session
• Review what will be done/covered during this visit​
• Review likely risk factors

Confirm diagnosis
•​ Spirometry pre & post
• Address concerns/questions/explain procedure
• Pre-approved spirometer
• Pre and post if necessary
• Written report produced
• History questionnaire

Smoking cessation
• Only thing that can prevent disease progression
• Use Fletcher graph
• Transtheoretical model – try to move patient to next stage of change
• Motivational counseling (McIlvain 1998) – Express empathy, Develop discrepancy, Avoid argumentation, Roll with resistance, support self-efficacy
• Develop tools for change with patient e.g. pro con list, set quit date if appropriate

​Medications
• Address concerns/questions
• Action of medications
• Beta agonists, Anticholinergics and side effects
​• Device technique
• Correct technique, Expiry date, Check how to know if empty
• Compliance; how often do they really use meds, fears, goals towards

Medication use and better disease control

Breathing Techniques / reducing shortness of breath
• Pursed-lip breathing
• Positions to reduce shortness of breath
• Energy Conservation

Lifestyle
• Importance of exercise and nutrition
• Compliance
• Discuss family / community support
• Sleep
• Pulmonary rehab

Follow Up (Visit Two)

• Address questions and concerns from the patient
• Assess stage of change, receptiveness
• Review goals
• Review strategies
• Discuss positive and negative results
• Acknowledge progress, not perfection
• Discuss implementation of changes/course corrections
• Provide encouragement

Smoking Cessation

Medication Review
• Device technique review
• Review medication usage

Spirometry

Management Plan Review
• Discuss exacerbations
• Vaccinations - Influenza and pneumonia
• ​Action plan
• Discuss action plan for exacerbations
• Discuss to what level the individual would be comfortable in put plan into action.
​Community Resources
• Discuss any community resources that may benefit patient and family.

Follow Up Session (Visit Three)

Address questions and concerns from the patient
• Assess stage of change, receptiveness
• Review goals
• Review strategies
• Discuss positive and negative results
• Discuss implementation of changes/course corrections
• Provide encouragement
