Cold Lake is in northeastern Alberta and is named after nearby Cold Lake. Canadian Forces Base Cold Lake is situated within the city’s outer limits.

Cold Lake was first recorded on a 1790 map, by the name of Coldwater Lake. Originally three communities, the City of Cold Lake was formed by merging the Town of Grand Centre, the Town of Cold Lake, and Medley (Canadian Forces Base 4 Wing) on October 1, 1996. Grand Centre was renamed Cold Lake South, and the original Cold Lake is known as Cold Lake North. Because of its origins, the area is also known as the Tri-Town. The City of Cold Lake has a population of approximately 16,000 people.

About The Cold Lake PCN

The Cold Lake Primary Care Network is committed to improving primary care services in the community of Cold Lake by implementing a collaborative approach to care between physicians, nurse practitioners, and other health care professionals.

The Cold Lake Primary Care Network currently has two locations. Our main clinic is located on the second floor of the Cold Lake Healthcare Centre, and our secondary (satellite) clinic is on the second floor of the Canex mall on 4 Wing Cold Lake.

Position Summary

The Nurse Practitioner, Primary Care (NP) will work within a multidisciplinary care team with the goal of optimal community wellness. The NP will work in a flexible, responsive manner to provide primary care services within the Cold Lake Primary Care Network. As a key clinician involved in the delivery of primary care services of the Cold Lake PCN’s operating strategy, the NP will work with a high degree of professional autonomy and judgement.

What you will do...
  • Primary care service delivery including but not limited to chronic disease management, prenatal care, and mental health/addictions
  • Identify and manage patients with chronic disease in collaboration with physician
  • Autonomous case management of patients with highly complex needs
  • Apply holistic approach to individual patient needs and develop health care plan in collaboration with family physician to support patient in their health journey
  • As per current best practice guidelines, ensure patients are assessed and screened
  • Actively lead/engage in patient navigation and clinical process redesign
  • Lifestyle counseling for patients with, or at risk for, a wide range of chronic and/or complex health conditions
  • Facilitate group programs including mental health & addiction programming as required
  • Participate in Cold Lake PCN and ongoing quality improvement initiatives
  • Work in partnership with Covenant, Alberta Health Services (AHS) when appropriate
  • Other duties as required within Nurse Practitioner scope of practice
What you need to succeed...
  • Master’s degree in nursing, with Nurse Practitioner certification Fully licensed and in good standing with regulatory body
  • Current BCLS certification for health care providers
  • Mental Health and Addictions experience an asset
  • Prenatal and Obstetrics experience an asset
  • 3 or more years of nursing experience, preferably in a rural primary care setting
  • Experience working within a multidisciplinary care team model
  • Experience working with the Indigenous population
  • Experience working with Addictions & Mental Health programming & principles
  • Knowledge of chronic disease management principles and processes using current best practice guidelines
  • Knowledge of EMRs and clinical process redesign is an asset
  • Knowledge of and interest in principles of a population-needs based approach, primary care, primary health care and the medical home model is an asset
  • Self-motivated to work autonomously to full scope of practice
  • Strong communication skills
  • Ability to recognize and resolve conflicts
  • Demonstrated high level critical thinking and decision making skills
  • Excellent time management skills
  • Expertise in clinical guidelines Ability to manage a varied caseload
