Prenatal Care

The Cold Lake Primary Care Network is here to help women maintain a safe and healthy pregnancy.

The CLPCN Registered Nurses, in consultation with the Nurse Practitioners, provide consistent and comprehensive prenatal care throughout each pregnancy to any unattached patient.

Please fill out and print medical history form below and bring to your first prenatal appointment. If unable to fill out prior, one will be available at first appointment.
