About The Cold Lake PCN

The Cold Lake Primary Care Network is committed to improving primary care services in the community of Cold Lake by implementing a collaborative approach to care between physicians, nurse practitioners, and other health care professionals.

The Cold Lake Primary Care Network currently has two locations. Our main clinic is located on the second floor of the Cold Lake Healthcare Centre, and our secondary (satellite) clinic is on the second floor of the Canex mall on 4 Wing Cold Lake.

Position Summary

The Mental Health Therapist will work as a member of a multidisciplinary team providing evidence informed and best practices in assessment, treatment, support and follow-up.

What you will do...
  • Work to full scope of practice providing comprehensive care to child, youth, and young adult populations
  • Provide psychosocial interventions specific to mental health and/or addictions to individuals using various therapeutic approaches
  • Compose progress reports for care team members (i.e. physician, school team, social worker, mental health navigator)
  • Provide appropriate mental health and/or counselling services focused on evidence-based practice standards
  • Intake screening and preliminary assessment including referrals, provisional diagnoses, consultation, and liaising with PCN providers, family physicians, community partners and psychiatry as needed.
  • Establish treatment goals, priorities and plans
  • Measure clients’ progress regularly and adjust treatment accordingly
  • Provide a seamless transition to AHS Addictions and Mental Health if required
  • Assist with program development and implementation aligning with best practices
  • Ensure the maintenance of accurate records in line with practice policy
  • Assist in developing and implementing patient self-management programs
  • Ensure continuity of service/care by liaising and collaborating with other providers
  • Participates in program development and quality improvement activities
  • Maintain professional requirements and ongoing active participation in professional development
  • Maintain current knowledge and skills in clinical practice
  • Offer training support to other team members and assist in their professional development
  • Act as a resource to PCN staff
What you need to succeed...
  • Master’s Degree in Psychology or Bachelors Degree in Social Work (with Psychosocial Intervention Authorization). Master’s in Social Work preferred. Other specialties would be considered, including specific registered counseling designations
  • Must have a minimum of 2-4 years recent clinical experience with child, youth, and adult populations
  • Must have excellent assessment skills
  • Must have formal clinical training and knowledge in child, youth, and adult mental health. CBT and Solution-Focused Therapy training considered an asset
  • Experience working with Child and Family Services, Health Partnership Initiatives, and school programs would be an asset
  • Demonstrated knowledge and skill in the assessment and treatment of children, youth, and adults with mental health and/or addictions concerns/diagnoses is required
  • Must have effective interpersonal and communication skills and the ability to work well with a range of community stakeholders and service providers
  • Certifications in: Applied Suicide Intervention, Non-Violent Crisis Intervention would be an asset
  • Must be registered with or eligible to be registered with specific regulatory body
  • Proof of a clean criminal record check with vulnerable sector is required
  • Completion of reference checks
  • Current Registration with relevant licensing body (submitted annually)
