Cold Lake was first recorded on a 1790 map, by the name of Coldwater Lake. Originally three communities, the City of Cold Lake was formed by merging the Town of Grand Centre, the Town of Cold Lake, and Medley (Canadian Forces Base 4 Wing) on October 1, 1996. Grand Centre was renamed Cold Lake South, and the original Cold Lake is known as Cold Lake North. Because of its origins, the area is also known as the Tri-Town. The City of Cold Lake has a population of approximately 16,000 people.

About The Cold Lake PCN

The Cold Lake Primary Care Network is committed to improving primary care services in the community of Cold Lake by implementing a collaborative approach to care between physicians, nurse practitioners, and other health care professionals.

The Cold Lake Primary Care Network currently has two locations. Our main clinic is located on the second floor of the Cold Lake Healthcare Centre, and our secondary (satellite) clinic is on the second floor of the Canex mall on 4 Wing Cold Lake.

Position Summary

The Practice Facilitator plays a key role in assisting clinic teams with improvement towards the pillars of the Patient’s Medical Home model of care. The role includes working with PCN team members, physicians, and clinic staff to advance their work processes related to patient access, patient attachment, panel management, patient screening, chronic disease management, and care planning.

What you will do...
  • Engage and develop relationships with primary health care teams and family physicians
  • Facilitate awareness and understanding of the concept of the patient-centered medical home (PMH) and the associated PMH pillars related to team-based care, panel management, preventative care, and enhanced patient access
  • Assist clinic teams to identify roles and develop work processes that improve efficiency and patient-centered care
  • Support ongoing team meetings, reinforcing improvement concepts, objectives, and methods
  • Assist with the identification and implementation of improvement projects
  • Mentor and coach clinic staff to implement projects and/or new/redesigned processes
  • Work with physicians and clinic staff to leverage EMR software and other documentation systems to support proactive patient care
  • Act as a resource to clinics respecting EMR optimization including providing training on advanced features and promoting process standardization and efficiency
  • Support clinic teams to gather data and report on process and outcome measures

  • Effective verbal and written communication skills
  • Excellent organizational and time management skills
  • Facilitation, problem solving, decision making and conflict management skills
  • Proficiency in using Microsoft Office applications
  • Demonstrated aptitude with EMR or similar software application
  • Ability to balance multiple competing priorities
  • Ability to support change management
  • Understanding of primary health care delivery organizations (including family physician practices) in Alberta
  • Undergraduate degree or a diploma in a related field of study, preferably a health discipline, is preferred. (i.e., LPN, RN, MOA or other allied health professional)
  • Valid driver’s license with access to a reliable vehicle for travel
