Prenatal Care

The Cold Lake Primary Care Network is here to help women maintain a safe and healthy pregnancy. The Cold Lake PCN Registered Nurses, in consultation with the Nurse Practitioners, provide consistent and comprehensive prenatal care throughout each pregnancy to any unattached patient.

Please fill out and print the medical history form below and bring to your first prenatal appointment. If unable to fill out prior, one will be available at first appointment. 

Cold Lake OB Questionnaire

What To Expect at Your Appointments
First Trimester (0-13 weeks)
  • First visit will include review of obstetrical/medical/family history, ordering routine lab work and early ultrasound
  • Maternal vital signs assessed each visit
  • Maternal weight measureed each visit
  • Urine samples are provided at the initial visit and then as needed (patient specific)
  • Physical will be done by the NP
  • Optional testing includes genetic screening (nuchal translucency, Invitae)
  • Dating ultrasound
  • Visits every four weeks with the RN
Second Trimester (14-26 weeks)
  • Routine ultrasound at 20 to 21 weeks
  • Fetal heart rate assessed each visit
  • Gestational screen & repeat lab work at 24-26 weeks
  • Fundal height measuring starts at 18 weeks
  • Visits every four weeks
Third Trimester (27-40 weeks)
  • Moms with Rh negative blood type will receive a WINRHO shot at 28 weeks
  • Referral to the delivering physician
  • Fetal movement counting
  • dTap vaccine recommended
  • Group B. strep swab at 35 weeks
  • Visits every two weeks from 28 to 36 weeks, and every week from 36+ weeks
